Pandemic Board Game: The Co-op Classic, Rules and Editions
Pandemic is a cooperative board game for 2 to 4 players, ages 8+ on the box: your team travels the world treating four diseases and racing to find all four cures before outbreaks, the cube supply or the player deck runs out. Everyone wins or loses together. The family on Amazon US also includes Pandemic Legacy: Season 1, two shorter Hot Zone boxes, and an expansion. In this game the deck of cards is the clock, and it's always running fast.

What the Pandemic board game is
Pandemic is a cooperative game. You and up to three others are a disease-control team. Four diseases are spreading across a map of the world, and your job is to find a cure for all four. Everybody at the table wins together or loses together.
What makes it tense is how many ways there are to lose. Z-Man's rulebook gives three:
- 8 outbreaks happen
- A disease needs more cubes than are left in the supply
- The player deck runs out when you need to draw
So the cubes are the map's fever, the outbreak track is the panic meter, and the player deck is the clock.
Box facts for the base game, from the listing and Z-Man's page:
- Players: 2 to 4
- Ages: 8+
- Play time: 45 to 60 minutes on the listing; 45 on Z-Man's page
- Co-op or competitive: cooperative
- Expansion needed: no
- Designer: Matt Leacock
How Pandemic plays
The rules below are in my own words, from the Pandemic rulebook Z-Man posts on its Pandemic page. For the exact wording, it's a free PDF.
Setup in brief
The board is a world map with 48 cities. Put one research station in Atlanta, home of the CDC, and all the pawns start there too. Each player gets a role card with a special ability. The rulebook lists 7 roles in the box, and you deal one to each player.
Then the map gets sick before anyone moves. Flip 9 infection cards: three cities get 3 cubes, three get 2, three get 1. That's 18 cubes on the board at the start.
Last, build the player deck with Epidemic cards spread through it. The rulebook sets the difficulty here:
- 4 Epidemic cards: Introductory, recommended for your first game
- 5 Epidemic cards: Standard
- 6 Epidemic cards: Heroic
I like a game that prints its own difficulty settings. I also like that "Heroic" is the word they chose, and not "Hard". It makes losing sound noble.
A turn in three steps
- Do 4 actions.
- Draw 2 player cards.
- Infect cities.
The actions are where the game lives. You can repeat any of them:
- Drive or ferry: move along a line to a connected city
- Direct flight: discard a city card to fly to that city
- Charter flight: discard the card of the city you're in to fly anywhere
- Shuttle flight: move between two research stations
- Build a research station: discard the card of the city you're in
- Treat disease: remove one cube where you stand
- Share knowledge: give or take the card of the city you're both in
- Discover a cure: at a research station, discard 5 cards of one color
Your hand limit is 7. Advice flows freely around the table, but the rulebook is clear that the player whose turn it is makes the call.
Infection, epidemics and outbreaks
At the end of your turn, flip infection cards equal to the infection rate and add a cube to each named city.
An Epidemic card does three things in a row. The infection rate goes up. The bottom card of the infection deck gets 3 cubes. Then the infection discard pile is shuffled and put back on top of the deck. That last step is the cruel one. The cities that just got sick are about to get sick again.
If a city would get a 4th cube of a color, it doesn't. It has an outbreak instead: every connected city gets a cube of that color, and that can chain into more outbreaks. Each outbreak moves the marker. At 8, the team loses.
Roles change the rules for one player
Each role bends one rule. The base rulebook's own example uses the Scientist, who needs only 4 cards of a color to discover a cure instead of 5. The Hot Zone – North America rulebook prints the Medic, who removes every cube of one color with a single Treat action.
That's the heart of the teamwork. On any turn, the right question is less "what can I do?" and more "whose ability fixes this?" In the rulebook's example, one player offers a city card, the Scientist flies over to take it, and the team cures a disease that turn.
How you win
Cure all four diseases. That's it. You don't have to clear the board. Once a disease is cured, treating it gets easier, and if you remove its last cube from the map it's eradicated and stops spreading entirely. Eradication is a bonus, not a requirement.
Lucho's take: "Four actions a turn and the whole map on fire. That's a Saturday at the shop in July, just with fewer trucks."
Pandemic editions on Amazon US right now
Our data for this page lists seven Pandemic items on Amazon US as of October 2026, all from Asmodee, which distributes Z-Man's games. The table above shows what each listing states. Here's what tells them apart.
Pandemic (base game)
The listing says 2 to 4 players, ages 8+, 45 to 60 minutes. It's the game the rules section describes. If you're buying one Pandemic box, this is it.
Pandemic Legacy: Season 1
The listing says 2 to 4 players, ages 13+, 60 minutes, tags it as a legacy or campaign game, and names the Blue edition. Z-Man's page says it's a stand-alone game by Matt Leacock and Rob Daviau. Over a campaign, you open sealed packets, characters gain scars, cities panic and diseases mutate. What happens in one game carries into the next.
Two things to know before you buy:
- Red or blue doesn't matter. Z-Man's page says the two editions have different covers and the same contents.
- It changes as you play. That's the design. Plan to play it through with the same group.
The 13+ on the listing is five years above the base game. Read it before you hand the box to a younger player.
Pandemic: Hot Zone – North America and Hot Zone – Europe
Two smaller boxes. Both listings say 2 to 4 players, ages 8+ and 30 minutes, and neither is an expansion. Z-Man's Hot Zone – North America rulebook shows how the game is trimmed:
- 24 cities instead of 48
- 3 diseases instead of 4
- 3 Epidemic cards
- Hand limit of 6
- Cure with 4 matching cards in Atlanta
- 7 optional Crisis cards, left out of your first game
Z-Man's Hot Zone – Europe page describes three diseases threatening Europe, four new characters and a mutation module. Z-Man also posts "Hemisphere" rules for combining the two Hot Zone boxes. If you want Pandemic in 30 minutes, start here.
Pandemic: Reign of Cthulhu
The listing says 2 to 4 players, ages 14+, 40 minutes, and it's a stand-alone game. Z-Man's page describes the same core play with a horror theme: seal four gates, stop cultists and Shoggoths, and keep an Old One from waking. The box adds sanity tokens and a sanity die. At 14+, it carries the highest age in our data.
Pandemic: Contagion
The listing says 2 to 5 players, ages 13+ and 30 minutes, and it's not an expansion. The listing doesn't describe how it plays, and the Z-Man page we tried didn't load, so I won't describe it. Check the box before you buy if you're expecting the cooperative game above.
Pandemic expansions
Pandemic: On the Brink
The one expansion in our data. The listing names Pandemic as the base game, lists 2 to 5 players, ages 8+, 45 minutes, and counts 7 roles and 8 event cards. Z-Man's page adds three optional challenges:
- Virulent Strain: one disease gets nastier as the game goes on
- Mutation: a fifth disease turns up in unexpected places
- Bio-Terrorist: a fifth player works secretly against the team
My one opinion for this page: get the base game to the table a few times before you buy On the Brink. Its challenges make Pandemic harder, and the base box already has three difficulty settings. If your team can't yet win at 5 Epidemics, you don't need a virulent strain.
Pandemic vs Forbidden Island
Both are cooperative games designed by Matt Leacock, for 2 to 4 players. Gamewright lists Forbidden Island at ages 10+ and about 30 minutes. The team recovers four treasures from a sinking island and escapes before it goes under.
Here's the table version:
- Want the shorter game? Forbidden Island or a Pandemic Hot Zone box, both around 30 minutes.
- Want more decisions per turn? Pandemic, with 8 kinds of action and three ways to lose.
- Youngest player is 8 or 9? Pandemic and Hot Zone list 8+. Forbidden Island lists 10+.
Who should skip Pandemic
Skip it if your group needs to beat each other. There's no individual winner. If someone at your table only enjoys a game when they finish ahead of you, Pandemic will feel like homework with a map.
Skip it if one player tends to run everyone's turn. Every co-op game has that risk. The rulebook's answer is that the active player decides, and it's worth saying out loud before you start.
Skip Legacy if you can't keep the same group together for a campaign, and skip Reign of Cthulhu for anyone under its listed 14+.
If you like Pandemic, look at these
Pages chosen by what they share with Pandemic: cooperative play and a clock that runs against the whole table.
- Mysterium: cooperative mystery, the clock is a pocket watch
- Hanabi: cooperative card game with limited talk
- Strategy board games: the wider shelf
- Outfoxed: a cooperative race for younger kids
For more cooperative games, see the board games section.
Start with four Epidemics. Heroic can wait.

Questions people ask
What is Pandemic Legacy?
Pandemic Legacy: Season 1 is a stand-alone campaign game by Matt Leacock and Rob Daviau. Z-Man's page says actions carry over from one game to the next: you open sealed packets, characters gain scars, cities panic and diseases mutate. The listing says 2 to 4 players, ages 13+ and 60 minutes. You don't need the base game to play it.
What's the difference between Pandemic Legacy Season 1 red and blue?
Only the cover. Z-Man's page says the Red and Blue editions have different cover art and the same contents. The listing in our data is the Blue edition. Buy whichever one is available, and don't buy both expecting two different campaigns.
What is Pandemic Hot Zone?
A smaller, faster Pandemic. Z-Man's rulebook for Hot Zone – North America uses 24 cities, 3 diseases, 3 Epidemic cards and a hand limit of 6, and you cure a disease with 4 matching cards in Atlanta. Both Hot Zone boxes in our data list 2 to 4 players, ages 8+ and 30 minutes.
Pandemic vs Forbidden Island: which should I get?
Both are cooperative games by Matt Leacock for 2 to 4 players. Gamewright lists Forbidden Island at ages 10+ and about 30 minutes, with a team rescuing four treasures from a sinking island. Pandemic's listing says ages 8+ and 45 to 60 minutes. For a shorter Pandemic, look at Hot Zone at 30 minutes.
Can you play Pandemic solo?
The base game lists 2 to 4 players, but Z-Man posts a free Solo Rules Variant PDF on its Pandemic page, and the Hot Zone page lists solo rules too. The rulebook itself is written for 2 to 4. Playing two roles yourself, as if it were a 2-player game, is a house approach, not an official mode.
Do you need the base game for Pandemic: On the Brink?
Yes. On the Brink is an expansion; the listing names Pandemic as the base game, and Z-Man's page says the base game is required. It lists 2 to 5 players, ages 8+ and 45 minutes, and adds 7 roles, 8 events and three optional challenges.
